I am pleased to formalize our offer of employment with you with The NHP Foundation (“NHPF”). We propose that DATE, be your first day as an NHPF employee, and our understanding is that you agree to that start date. Your position will be FUNCTION , you will work out of NHPF’s office in STATE, and your duties will be those reasonably required by the NHPF as communicated to you by your supervisor. The NHPF has agreed to provide you with an annual salary of $AMOUNT pro-rated for YEAR. You will also be eligible for a bonus based upon your performance and the programmatic and financial success of NHPF (also pro-rated for your first full year). Our year for pay purposes runs from January 1 to December 31.

Additionally, we will provide the following benefits:

1. Vacation: XX weeks per year (earned at the rate of 2.88 hours per bi-weekly pay period);

2. Personal Leave: 2 days per year (expires at the end of each year);

3. Sick Leave: 10 days per year (expires at the end of each year);

4. Coverage under N HPF’s health plan, and currently 90% of the premiums are paid by NHPF (this percentage is adjusted each February);

5. Other Insurance plans: NHPF currently pays 100% of the premiums for: • Short-Term Disability • Long-Term Disability • Term Life

6. Flexible Spending Plan: Eligible to be reimbursed for health or child care costs on a pre-tax basis;

7. Long-Term Care Insurance: Eligible to participate after one full year of employment;

8. Paid holidays as designated by NHPF; and

9. 401(k) Retirement Plan. NHPF will make a 20% match on your first $10,000 in contributions per calendar year.
